Checklist of the required documents:

1. LoI Acceptance Letter or Accreditation Certificate

2. Campus Regulation / Statute

3. Decision minutes of the last two renewals of Campus Management Committee

4. Meeting minutes of Campus Management Committee of the last two years

5. Decisions of Campus Management Committee regarding establishment of unit for

EMIS with approved budget and human resource allocation in the FY 2071/072

6. Audit reports of the fiscal years – 2069/070 and 2070/071

7. Annual report of the fiscal year 2071/072

Note: Should there be need for revision in the Statistical Data Form 2071/072 submitted to UGC earlier, please resubmit the updated data form with explanation and necessary verification documents.
